Hours - 35 hours per week (+ 5 hour paid lunch)

Who are we looking for?

We are search of an inspirational Practitioner to work as part of the Recovery Near team, within the Treatment team, to ensure that service users successfully achieve their planned objectives, by delivering effective interventions and taking a multi-agency approach to service delivery to ensure that the right services are available at the right time. You will ensure that front line delivery meets the contract specification and is delivered to a high standard.

The successful candidate will have a minimum of 12-months experience of working in multi-agency setting, particularly within the Substance Misuse or associated field, as well as line-management experience. The ideal candidate will have excellent communication skills and the ability to engage with and motivate challenging service users from a diverse range of backgrounds, often with multiple and complex needs. Lastly, we are looking for an individual with experience of working as part of a team with a holistic approach to service delivery, particularly with experience in developing and delivering 1-1 psychosocial interventions and group activities.

 

Duties and responsibilities include but are not limited to:

 

  • Design and deliver effective interventions which are solution focused and practical.
  • Undertake thorough assessments of need and develop care plans where required.
  • Support service users to make progress against their objectives by providing tailored pathway interventions and making supported referrals into other agencies.
  • Record, monitor and review progress against individual and service objectives in order to demonstrate impact and tangible outcomes.
  • Empower and motivate service users to identify and achieve desired outcomes. Actively engage with service users in decisions that affect them ensuring they have input in shaping the nature of services delivered to them through involvement activities.

An Enhanced DBS check will be required for this role.

Full UK driving licence or access to a vehicle is desirable.
